25 Verification of Need for A Reasonable Accommodation
Needed to legally fulfill the request process for a reasonable accommodation for people with disabilities that are not accommodated by the landlord’s rules, policies, practices, or services associated with housing. This form should be completed by a doctor, other medical professional, or other knowledgeable third party, to confirm the applicant/tenant’s disability status.
