27 Verification of Need for A Reasonable Modification
Needed to legally fulfill the request process for a reasonable modification to the premises for people with disabilities that are not accommodated by the landlord’s rules, policies, practices, services associated with housing, or the dwelling accessibility. This form should be completed by a doctor, other medical professional, or other knowledgeable third party, to confirm the applicant/tenant’s disability status.
